What the columns mean
Seven fields turn a list into a review system.
Keep each entry short. Every field should help you decide what to do before the mistake repeats.
2. Error type
Use a small set of labels: concept gap, interpretation, process, careless error, strategy, or timing.
3. Root cause
Name the exact step or decision that changed the result. Avoid vague notes such as “I was careless.”
4. Correct approach
Write the method in one or two sentences, focusing on the step that should replace the failed one.
5. Next action
Choose one concrete task: redo the problem, complete a short skill set, or apply a checking routine.
6–7. Review date and status
Schedule the next check and mark the row new, review again, or secure based on your retest.

What to log
Include wrong answers and correct guesses.
A correct guess can hide the same weakness as a wrong answer. Add any question you could not explain, solved with an unreliable shortcut, or answered after running out of time.

Weekly review
Look for patterns before adding more practice.
